Q12 or in general

How do you tell what indicator you should take into consideration? In Q12, the beginning of the sentence is "when", which is an indicative word. Why isn't the first sentence also a condition? Of course, in hindsight, we know it's not. But are there rules to identify quickly within 1:30 min?

Like, in the explanation, she analyzed almost every sentence afterward. Why isn't "I read a novel set in a city I know well" a sufficient and "I must see that the writer knows the city as least as well as I do" a necessary condition too?

I diagrammed that part of the sentence as well, and I found that it didn't affect the passage in any way. It was just extra info there. So I would assume just diagramming everything is the safest way to go.
