Can this be explained

In reading this I understood the stimulus, but I had a hard time with the question and understand what each answer choice was exactly stating so I was wounding if this could be explained further.

We are looking to find an answer choice that tells us that it is not ethical under the circumstances for a doctor to report the man to police, without contradicting anything in the passage. The right answer would therefore be something that would enable us to say "therefore given what we know, it is not ethical."

C does exactly this; it tells us it is never permissible for doctors to report to police, unless in the case of gunshot wounds.
