Where is the idea of "good journalistic practice" introduced? Going to the supposed correct choice A, why would the three situations provided mean that there is "never" a good time to use this phrase?

Hi, these three situations are collectively exhaustive, or at least the author thinks they are. We are only being asked what their conclusion is, which is that it shouldn't be used. This is not an exact word for word match for poor journalistic practice, but it is expressing the same idea.
